These USB wall chargers have inadequate insulation between the high-voltage AC input and the low-voltage USB output. This can cause a lethal electric shock when you touch the USB connector while the charger is plugged in. One death has occurred. The chargers were sold without UL safety certification.

Why This Happens: Technical Breakdown
A safe USB charger has a transformer with multiple layers of insulation separating the 120-volt wall power from the 5-volt USB output. The recalled chargers use a single thin layer of insulation that can break down over time or with minor physical damage.
When the insulation fails, the full 120 volts from your wall outlet is present on the USB port. If you plug in a phone and touch the metal connector or the phone’s frame at the same time, you receive a potentially fatal shock — especially in wet environments like bathrooms or kitchens.
